Place the CPT tubes into the plastic tray, cover with absorbent pad, and close the plastic tray.

Place the tray inside the biohazard bag provided and seal the zip closure. Place the completed test request form into the back pocket of the biohazard bag.

Remove the diagnostic envelope and FedEx airbill from the white cardboard box. Place the plastic tray sealed in the biohazard bag into the white cardboard shipping box.

Close the white box and place it inside the red FedEx lab pack envelope. Seal the envelope.
