Collect the patient's blood into the two blue/black CPT tubes using a 21 gauge needle. Allow the tubes to fill completely. Minimum requirement is one full tube. Invert the tubes 5 to 6 times to ensure adequate mixing.

THE CPT TUBES SHOULD NOT BE CENTRIFUGED, REFRIGERATED OR FROZEN!

Label the tube clearly with the patient's name. Peel the requisition labels from the test request form and place it on the tubes.

Make sure the test request form is completely filled out, including the date of collection and the test being ordered. Incomplete forms will delay test results.
